How to add custom disk partition for centos6.8 in foreman partition tables

Hi,

We have refer the URL: http://projects.theforeman.org/projects/foreman/wiki/Dynamic_disk_partitioning to add our custom disk partition in centos 6.8 unattended installation through Foreman, We've confusion about to add this disk partition on the Provisioning templates,

whether we need to add a separate file or need to input the contents on my selected provisioning templates, kindly share the detailed steps to complete the disk partition, we are really need your assistance at this moment because we didn't get any reply from our another post about the ubuntu pkg issue: http://projects.theforeman.org/issues/16504

Disk partitioning schemes can be added under the Hosts > Partition table menu, and selected when you create the host.
