update foreman_one to 0.4

Replace plugin upgrade/install test to fix Bundler errors

When a plugin is upgraded and the version changes, the `bundle show`
command always failed as bundler.d/ had already been updated, but the
Gemfile.lock and vendor/ area was out of date.

Error from an upgrade where 1.0.1 was currently installed, but...

Set HOME to ~foreman as rb-readline requires it

fixes #7970 - add and prefer /usr/bin/foreman-ruby symlink

Use foreman-ruby for bundle calls especially, so it allows the default Ruby
to vary separately from Foreman's. Set foreman-ruby to ruby1.9.1 explicitly
on Ubuntu 12.04.
